\nHolding out hope for people suffering from ACHALASIA, a
\nrare disorder of Esophagus causing difficulty in swallowing food, a
\nPer-Oral Endoscopic Myotomy, an advanced Endoscopis procedure has been
\nperformed for the first time at SRM Medical College Hospital and
\nResearch Centre, Kattankulathur, COO Dr. Ram Natesan has said.<\/p>\n
The operation was performed by SRM Medical College Hospital and
\nResearch Centre\u2019s interventional endoscopic gastro enterologist Dr N A
\nRajesh under the overall supervision of chief surgical gastro
\nenterologist Dr. A. Rathnaswami.<\/p>\n
\u201cA 45-year-old gentleman presented himself before us with difficulty
\nin swallowing he has suffered for over three months. He had difficulty
\nin eating and complained of vomiting of food immediately after eating.
\nHe was evaluated with Upper GI Endoscopy, Barium Swallow and
\nOesophageal Manometry and was diagnosed to have a rare oesophageal
\ndisorder known as Achalasia Cardia. We gave him the option to correct
\nhis problem endosopically and he agreed, knowing that we are going to
\nperform this procedure for the first time. He underwent Per-Oral
\nEndoscopic Myotomy (POEM) procedure in SRM Hospital recently,\u201d Dr.
\nRajesh said.; .<\/p>\n
Elaborating, Dr Rajesh said the procedure was conducted without any
\nglitches and the patient tolerated the procedure well. He recovered
\nwell, was given fluids the next day and regular diet the third day.
\nOne month after the procedure now, he is eating well without vomiting.<\/p>\n
The procedure was first introduced in 2008 in Japan and is currently
\ndone only in a few advanced endoscopic centres in India. This
\nprocedure requires, apart from specializaed endoscopic equipment, a
\nskilled endoscopist with adequate training in therapeutic Endoscopic
\nprocedures.<\/p>\n
\u201cSuch advanced endoscopic facilities and skill and knowledge of
\ntherapeutic endoscophy is available in Gastroenterology Department of
\nSRM Medical College Hospital and Research Centre, thanks to the
\nadministrators and management which evinces keen interest in assisting
\nus to develop the endoscopic centre as a \u201eCentre of Excellence in
\nTherapeutic Endoscopy,\u201d said Dr. Rajesh.<\/p>\n
Indeed this procedure has been done at the SRM Medical College
\nHospital and Research Centre for the first time. At present only three
\ncentres in Chennai \u2013 GLOBAL, GEM, and Medindia Hospitals in private
\nsector perform this advanced Endoscopic Procedure.<\/p>\n

ABOUT ACHALASIA:<\/p>\n
ACHALASIA is a rare disorder of Esophagus causing difficulty in
\nswallowing food. It is caused by damage to the nerves that supply the
\nesophagus. This results in failure of relaxation of sphincter muscle
\nin the lower end of the oesophagus the valve which usually allows food
\nto go from food pipe to stomach. Failure of relaxation of valve
\nprevents food to go into the stomach and the patient has difficulty in
\nswallowing.<\/p>\n
Achalasia can be diagnosed by (a) Barium Swallow- X-ray of chest taken
\nafter swallowing of a liquid solution called Barium. (b) Upper GI
\nEndoscopy \u2013 A fliexibe tube with light and camera inserted through
\nmouth to inspect esophagus and stomach. (c) Esophageal Manometry \u2013
\nGold standard in diagnosis, measures esophagus contraction and LES
\nrelaxation.<\/p>\n
TREATMENT:<\/p>\n
Achalasia treatment is focused on forcing open the non-relaxing valve.
\nThis is achieved by dividing the sphincter muscle- the Valve. This
\ncutting of the muscle which remains tight is done by surgery, and it
\nis known as Heller\u201fs Cardio Myotomy. This procedure is done over the
\nyears by minimally invasive Laparoscopic surgery and remained as Gold
\nstandard for this condition over the years. POEM is a novel procedure,
\nwhere the cutting of the sphincter can be achieved endoscopically
\nwithout any incision over the body. That is without Laparoscopic
\nsurgery.<\/p>\n
In Per Oral Endoscopic Myotomy (POEM) endoscope is inserted through
\nthe mouth and a tunnel is created between the lining of the oesophagus
\nand muscle, by making a hole in the lining membrane- the mucosa.
\nSubsequently the muscle at the lower of oesphageal sphincter cut
\nendoscopically. Finally, the opening in the lining membrane is closed
\nwith clips.<\/p>\n
POEM procedure offers long term symptomatic relief comparable to
\nsurgical H Cardiomyotomy.<\/p>\n
It does not require any abdominal incisions and requires minimal
\nHospital stay. POEM can also be done in patients who failed to get
